Report a Crime

We have created this online reporting form which allows you to report nonemergent incidents to the Sparta Police Department. These reports are not monitored 24/7 but Officers will review each online submission as a regular call for service and will follow up as needed. In order to ensure we provide the best possible police service, the following guidelines should be used:

  • The incident is not an emergency
  • The incident is non-violent and not currently in progress
  • The incident occurred in the City of Sparta
  • The most current and accurate information must be provided for an officer to contact you

Can be used for incidents to include but not limited to:

  • Harassment
  • Fraud/Financial Crimes
  • Identity Theft
  • Theft
  • Retail Theft - If the suspect is not in custody
  • Threats
  • Property Damage

We will NOT accept the theft of medications via online report. If you had medications stolen you must report it in person.

Report an Accident

Complete this Wisconsin Driver Report of Accident if:

There was $1,000 or more damage to any one person's property; Anyone was injured; There was $200 or more damage to government property, other than vehicles.

 

Do not complete this Wisconsin Driver Report of Accident if a law enforcement officer completed a Wisconsin Motor Vehicle Accident Report.

 

Voluntary Statement Form

If asked by an officer to complete a statement form, please print and complete this Voluntary Statement Form.

Worthless Checks Packet

If you need to report receiving a non-sufficient funds/worthless check, please print and complete this Worthless Check Packet. Once completed, the packet should be turned into the Sparta Police Department.
